Purpose
Practice X provides this patient portal to let you access your medical records and billing transactions online. You can view health-related information and communicate with our staff via secured messaging. By logging into the patient portal, you signify your agreement to these terms of use. If you do not agree to these terms, do not use the patient portal. Please note that the information provided is not a substitute for the advice of a personal physician or other qualified health care professional. Always seek the advice of a physician or other qualified health care professional with any questions regarding medical symptoms or a medical condition.
Security and Access
Access to the patient portal is enabled by usernames and passwords and additional security measures as may be implemented from time to time by Practice X. You shall maintain your username and password in strict confidence. In no event shall you share your username or password with any third party or allow another person to access the patient portal using your username and password. You shall immediately notify us if you have any reason to believe that your username or password has been lost, compromised, or misused in any way. You are fully and solely responsible for any and all use of the patient portal under your username and password.
Use of Patient Portal
You may use the patient portal to view information displayed from your medical records and billing transactions. You may also authorize other users of the patient portal to view such information through the "Act for a Family Member" feature. However, the patient portal does not necessarily display all information found in the medical records and billing transactions. If you think that certain information displayed is inaccurate, you can request our staff to make an amendment. If you are authorized to access another user's health information, you agree to protect the confidentiality of this information and comply with state and federal privacy laws that may prohibit the disclosure of health information without the express written authorization of the person who is the subject of the health information.

The patient portal contains information and materials supplied by Practice X, its affiliates, its licensees and other sources. All content is protected by United States and international copyright and trademark laws. The information provided in the patient portal is for your own personal use. You may not modify, copy, reproduce, republish, upload, download, post, transmit or distribute information or materials, including code and software, from the patient portal for commercial or business use. Permission to reprint or otherwise reproduce any information or materials contained in the patient portal, in whole or in part, for any use, is prohibited unless you obtain Practice X’s prior written consent.
Disclaimer
The patient portal is provided "as is," with all faults, with no representations or warranties of any kind, either expressed or implied, including, but not limited to, the implied warranties of merchantability, fitness for a particular purpose or non-infringement. You assume total responsibility and risk for your use of this patient portal. No oral or written information or advice given by Practice X or its authorized representatives shall create a warranty of any kind.
Limitation of Liability
Practice X is neither responsible nor liable for any direct, indirect, incidental, consequential, special, exemplary, punitive, or other damages (including, without limitation, those resulting from lost profits, lost data, or business interruption) arising out of or relating in any way to the patient portal, whether based on warranty, contract, tort, or any other legal theory and whether or not advised of the possibility of such damages. Your sole remedy for dissatisfaction with the patient portal is to stop using it. Applicable law may not allow the exclusion or limitation of incidental or consequential damages, so the above limitation or exclusion may not apply to you.
Governing Laws
The laws of XXX will apply to any disputes arising out of or relating to these terms. All claims arising out of or relating to these terms or the Services will be litigated exclusively in the federal or state courts of XXX, and you and Practice X consent to personal jurisdiction in those courts.
